Chiropractic Initial Consultation

Service Description

An initial consultation consists of a 20 minute discussion of your health history followed by a 40 minute chiropractic examination, and a 15 minute treatment. The examination is a functional assessment which identifies limitations in range of motion, joint restrictions, muscular imbalances, nerve dysfunction, and orthopedic tests to rule-in and rule-out specific health conditions in order to arrive at an accurate clinical diagnosis. As a regulated health professional and chiropractor, Dr. Jenna is qualified to diagnose your condition and discuss the findings of your orthopedic examination (this is called a "report of findings"). Your report of findings discussion includes: (1) going over any medical imaging reports (x-rays, CT scans, MRIs, or ultrasounds) and bloodwork reports, (2) your chiropractic diagnosis, (3) a plan of management, (4) and an honest discussion of the prognosis of your condition (whether or not your condition warrants chiropractic/acupuncture care). Once you have signed the Informed Consent form, you will then receive your first treatment this session. Please thoroughly review the INFORMED CONSENT form before your first appointment. For your first appointment, please bring in your filled out NEW PATIENT APPLICATION form, and 1-2 OUTCOME MEASURE forms relevant to your primary areas of complaint.
